Please make sure that you fill in all of the fields below before you click the payment button.  When you place an order with me (either via email or through my Ordering or Subscription pages), I will send you an invoice.  The invoice contains a list of all the items you have ordered.  IF you expressed interest in paying for your order via PayPal, then your invoice will also contain the following additional information: Product Total, Shipping Total, and a Confirmation Code.  You can then use this form to pay for your order using a credit card or existing PayPal account balance.
